[EPS Options] When saving Photoshop files as EPS, select Mac 8-bit preview or TIFF (for Windows), select Binary encoding, DESELECT Include Halftone Screens, DESELECT Include Transfer Function and DESELECT PostScript Color Management.

Do not save Photoshop EPS files with JPEG encoding or JPEG previews, as they may not separate correctly or may not image at all.
